I want to keep #nostr pure.
I've blocked a lot of them, too. They have their own relays.
But I try to differentiate between someone who occasionally posts something erotic in a discrete manner with NSFW tags, and someone replying to me with penis or pestering my male followers with their OnlyFans.
I can block labeled items and let the other stuff through.
I usually go like this: if I see a good #note or a fantastic #reply, I make sure to follow that #pleb right away.
And then I proceed and follow everyone he follows (just click on tue follower count on his profile). Itβs much easier to clear the spam and/or not good content then patiently wait.
I got great results with this so far.
Good strategy, should be implemented.

I'll add about five per week and have to unfollow one or two, in full panic, and then block them from my relay.
I'm so slow that my follow/follower ratio is collapsing and my feed is sort of dead. π
I like my feed to stay a little quiet so I dont get sucked into too many nonsensical ways of spending my time. I have a soft spot for funπ And I'm always rotating things around, maybe test my memory. I don't follow some of my favorite people & just keep a list in my head. I try to gage what/how often someone is posting before I follow/unfollow.

I started a year ago trying to circumvent the identity problem of Nostr. I am not a technical person. I tried to follow people I considered the most logical. I did that for a while without significant results. I then discovered the web of trust in coracle and went fishing logical people on my web of trust. That worked but my web of trust became very complex and the process of verifying the logicality of people pretty tedious.
Nowadays I'm following people that cause me curiosity (and fishing some of them in my web of trust feed) and it works great.
But I agree with some of the proponents of following everyone and then filtering. You could find some gold Nuggets with that process. Maybe you could use the 2 mentioned methods with 2 separate accounts or make a combination of both
Well, I already have two lists, basically because I use my npub feed for discovery for my relay.
I sometimes find people to follow-back here and test-run them for a few days:
