Logseq plus syncthing
Completely self hosted. Sometimes quite basic, but the freedom is worth it.
Seconding this, I use LogSeq at work with notes tracked in a git repo with GitCrypt. Works really well.
GitCrypt enables me to share my pages but keep journals private.

Obsidian. It's not fully open source but at least your notes are in an open format

Obsidian really is the best option. You can sync with a private git repo but IMO paying for sync is money well spent, the product works perfectly.

Standard Notes
Assuming you want open source for your private notes, then NotesNook and StandardNotes (premium SN is not open) are great options. Personally I prefer SN.
If you already use vim/nvim, then continue using and sync with SyncThing which works perfectly. Only downside is for phone you have to use a different text editor such as Markor.
Obsidian is very slick, high functionality and seamless, BUT it is not open source, so I wouldn't want my private notes in there.

I have a few, those being Standard Notes, Logseq and Notesnook.
Started on Standard Notes, but wanted more “functionality”. I tried to run logseq for a while. The mobile app was too restrictive. Tried obsidian and it didn’t work well for me. Eventually landed back on Standard Notes
Joplin, plus self-hosted web server. Is open-source, even the server code (you can also use dropbox etc.).
Covers all your needs, plus it even supports e2ee.

Alternatively use a chat program that allows msg to yourself, like saved msg on tg.
Obsidian + syncthing.
Not sure how to do it conflict free.
