Many of them are just people consolidating inputs, completely normal.
The inscription bs is usually at 1sat/vb or 4sat/vb; usually 10000 sat taproot outputs + big witness size.
Easy to spot.
I just followed someone there from Following+ before it loaded my follow list (this is an assumption).
This erased my contact list and replacing it with that single follow.

The reality is that anyone can push notes to relays, anyone can generate pubkeys, and anyone can continue to bother you.
You are in charge of filtering here, don't expect an authority to do it for you.
You can:
1) Only subscribe to paid relays that are willing to do moderation.
2) Use filtering tools to hide notes from people you don't follow.
3) Block/mute users that bother you.
I don't know what else to tell you.
*You* can filter an asshole. Will can't do it for you.
They're free to be assholes, and you're free to block them.
That's the entire point. We need better filtering controls on clients at the moment.
