Depends on your client. The only one I know for sure that allows it is Amethyst. Are you using Damus or something else?

On Amethyst there is a little eyeball with a triangle that'll let you mark it that way when you create a note. ๐
If you put #nsfw in your post, Amethyst will automatically consider it sensitive content. There are a few other hashtags it does this with, too. It will do so regardless of what client you posted from.
If you want to mark your post as sensitive content without adding that hashtag, you will need Amethyst as your client.
You self label it anything you want with a hashtag

This will also be helpful and safe for nsfw creators to grow and thrive here.

Eh, maybe...but I suppose it let's people have a choice. If you want to post adult content as well as be social with those who would rather not see it it's a compromise. As long as it involves free choice from all angles I don't mind.
I just found a way to disable it in the settings, so I did that. 
I have all of those off. It doesn't change when a poster elects to mark it as nsfw
