Great breakdown of a national electrical grid in general if anyone is interested

There’s definitely no harm to them

The issue is definitely a real one

But I’m skeptical about how well they work and they also come with a lot of preconditions

Furthermore they’re expensive, so I don’t push it too hard on my patients

Hopefully something takes off for atropine…interesting topic though

I only really understand your first question

If you have eye strain your optometrist should consider doing cyclopentolate refraction or using prism. Your case history, cause of strain and has a recent change in your prescription prompted it all matter. Poor sleep and stress can cause it too.

Prism corrects binocular vision. Remember you have 2 eyes but only see 1 image. There’s an entire network of muscles and nerves that work together to ensure you see one image.

I have prism in my own lenses as I get some eyestrain sitting in my office all day. It’s not a big deal to get either and can make a major difference to your eyes

Prism has other uses too btw

So regarding your last sentence, yes we are incentivised ofc but this is where I defend optometry

Your eye development is mostly complete when you’re 8 years old and there is still some change until you’re 18 or so. So no matter what you do really, the trajectory of your vision is essentially set in stone

Furthermore glasses aren’t like a drug or a crutch.

They aren’t like a drug because the prescription itself does not reflect the quality of your vision. It reflects the quality of your vision vs what’s considered the standard but…—> fundamentally your quality of vision is what you can see with the sharpest prescription given to you —> some people could have a small prescription and terrible vision with or without glasses. For example a blind person’s best vision may be achieved with simply +1.00 but the issue is the health at the back of his eye to where the light is being bent —> Glasses are like a telescope, they bend light onto the part of your eye that is most sensitive to seeing, this is called your macula and it’s the quality of this that’s important. This is basically all determined genetically

They aren’t like a crutch because there’s not a large amount of muscle needed to see. It’s mostly due to the shape and liquids of the eye and once you’re older than 40, the few muscles involved start to wane more and more anyways. You wouldn’t miscorrect a telescope to protect the magnification inside and likewise you should wear a fully corrected prescription. The potential protecting of muscle is negligible.

Also some people think glasses make their vision worse but they may not have realised how bad it was before they got them. Even though this sounds salesy I fully stand by it and everything else

The degradation of your near vision in your 40s is called presbyopia.

Some people in their 40s don’t need any glasses because their eyes are NATURALLY a bit short sighted but only a bit so that they don’t need glasses for far away. There are a few other minor exceptions. But once everyone turns 60 and the presbyopia gets worse, basically everyone needs them

So regarding your last sentence, yes we are incentivised ofc but this is where I defend optometry

Anti reflective coating can help but it’s not necessary unless it’s +-3.00DS imo or if you struggle night driving. It’s important to know anti reflective coatings only reduce glare that originally comes from the lens, not glare fundamentally

UV filters definitely reduce the onset of cataracts. People who use sunbeds have a much higher chance of developing pre mature cataracts due to UV exposure. However cataracts are an unserious condition and there may be negative consequences to not being exposed to UV…that I don’t know

Other filters put on lenses are usually chosen by the patient - tints, polarising, transitions etc and nothing to worry about.

I think blue light filters are a scam btw, too many leaps of thought and so so little real evidence. The best thing you can do for your mind and eyes is to just turn your phone off when it’s dark lol

Honestly the simplest advice I can give you is wear glasses when you need them and don’t worry too much about anything else. I’d be very healthy conscious about my diet but don’t sweat the small stuff when it comes to your eyes. :)

Optometrist here 👋 🇮🇪

Frames are made of terrible quality nowadays and essentially have built in obsolescence to last 4 year years max, and most shops recommending changing frames every 2 years

You use to be able to have frames made that would last your whole life and be constantly tailored but they no longer exist. You’d update the lenses when there was a significant change in the prescription

The quality of lenses themselves have probably improved though. Varifocals are much better than bifocals for example

Fun fact about the greatest county in #Ireland

The O'Reilly coin of Cavan is a fascinating piece of Irish history. The O'Reillys, a prominent Irish clan, were the kings of East Breifne, which roughly corresponds to modern-day County Cavan. What sets them apart is their unique economic practice: they developed and minted their own currency, which was quite unusual in Ireland at the time, where bartering was the primary means of trade

The O'Reillys began minting their own coins following the introduction of English coins to Dublin by Henry IV in the early 15th century. Their coins borrowed heavily from the design of these English coins, indicating a strategic adoption of a familiar monetary system to facilitate trade and establish their economic power

The O'Reillys' currency was so successful that it became widely circulated, and existing examples of their coins are highly regarded in Ireland. This economic innovation allowed the O'Reillys to control a significant portion of the trade in their region, further cementing their power and influence in East Breifne

Despite their success, the O'Reillys faced challenges. Two acts of Parliament in 1447 and 1456 outlawed the production and usage of "Reilly's silver," which was becoming increasingly prevalent. This suggests that the O'Reillys' economic power was seen as a threat, and efforts were made to curb their influence

The O'Reillys' coinage is a testament to their ingenuity and their ability to adapt to changing economic landscapes. By creating their own currency, they not only facilitated trade within their kingdom but also asserted their sovereignty and economic independence, making them a unique and significant part of Irish history.

#Pluribus is good fun

70% of the brain is cholesterol

Anti-cholesterol sentiment grew stronger when we went off the gold standard in the 1970s for reasons need not be explained here

Statins are a highly potent drug that should not be prescribed in the amount they are…crazy how many people in Ireland are on them

I read gun germs and steel recently. One the main reasons the Spanish conquered South America was their stronger immune system which without question came from their ability to domesticate animals first in Europe. They obliterated the populations there because of infectious disease, not so much war and conquest.

Meat makes you stronger in every single way.

There’s also the bio-Leninist aspect of it. Only rich people ate meat in the last few centuries, so instead I should eat what my peasant ancestors ate…

I know a guy that has spent his entire adult life in academia, never had a job, not even a part time job, because his father paid for his tuition.

I knew him when we were both in our 20's, and now, 10 years later, he rings me up out of the blue and asks if I want to go for a drink, I say sure.

He's still in college, as stated earlier, but I'm not really interested in browbeating him about that, so I just keep the conversation light, catch up, see what he's been up to.

This man keeps dragging the conversation back to his father.

His father rented his son's room out to a tenant. This is the room in the house that the man in question grew up in, and bear in mind that this man is in his 30's now.

He keeps addressing the father in this condescending language - boomer bitch etc

He's practically frothing at the mouth, and the level of vitriol that he has for his father is very unsettling.

I try my best to steer the conversation away from his father, as it seems obsessive, but try as I might, it always ends up back there, so I decide, fuck it, and ask questions about the father for a while.

As it turns out, the father had kept his son's room, perfectly preserved as it was when he first left for college, throughout the man's entire 10+ years in academia. In those 10+ years, the man barely visited his family home, and when he did, it was to mooch off them and get his laundry done.

As I press further in my questioning, the man reveals to me, to my horror, that the only reason that the man's father has rented the room out is that his wife has passed away. She was his father's lifelong companion and was one of the only people that the man's father really talked to.

It's blindly obvious that the man's father is unbearably lonely, sitting in an empty house, and simply wants some company.

He makes a rational decision to rent the room, that his 30 year old son never uses anyway, in an attempt to stave off his loneliness,

Instead of being there for his father through a desperate time of need, his 30 year old man child son is having hissy fits about his father to anybody who will listen, because he rented his room out.

I finished my drink, told the man how utterly despicable he was, and that if he hasn't grown up these last 10 years, he's not likely to do it at all, wished him all the best and never spoke to him again after that.

Last I heard he's doing something with Antifa now.
