I think the key difference between "censorship" and "anti-spam measures" is who does it.
Given nostr design, anyone can run their own relay and implement any kind of policies for themselves and its' users, there is no way to enforce it for all nostr users because there is a choice for an end user.
So far there is no central authority deciding who are allowed to use nostr and who aren't.
