There is no Nostr source, each software is completely independent from the others. Nostr is just a loose set of idioms they use to talk to each other. The NIPs are just documentation on the specific idioms used that exist to allow new people to write new interoperable software.
Is that what causes the inconsistencies? If I hop on my account, the dates, time, follows have changed slightly. Is it because I've moved to a different Nostr relay and the operability of the server is different (eg slower or further away physically) from the last server I was on?

You don't underatand how protocols work. Anyone can fork the standard and improve it if they can't get a nip through. It does not require consensus like bitcoin does.
